LASEK

1. Service includes LASEK procedure for both eyes including surgeon professional services. 2. The final net amount may be reduced after applicable discounts, such as Senior Citizen or PWD benefits. 3. The procedure is performed under local/topical anesthesia. 4. Facility fees and diagnostic tests are billed separately and are paid directly by the patient. 5. Post-operative medicines and three post-operative follow-up visits within six months are included. 6.
