1. Service includes diagnostic screening and eligibility assessment only. 2. Final surgical approval depends on full clinical evaluation by the ophthalmologist. 3. Fees vary based on number and type of diagnostic tests required. 4. Screening does not guarantee surgical qualification or specific outcomes. 5. Additional tests may be required for borderline or complex cases. 6. Dilation and repeat measurements may be necessary for accuracy. 7. Surgery, medications, and procedure fees are not included.
