OVERVIEW
EVO VIVA ICL is an Extended Depth of Focus phakic intraocular lens that improves distance and intermediate vision while preserving the natural cornea. It offers a reversible option for selected patients with moderate to high myopia. Reading glasses remain necessary for close or near tasks.
TERMS AND CONDITIONS, INCLUSIONS, AND EXCLUSIONS
1. Service includes EVO VIVA ICL implantation for one eye and surgeon professional services only.
2. The published price refers to the surgeon’s professional fee for one eye only. The final net amount may be reduced after applicable discounts, such as Senior Citizen or PWD benefits.
3. If the procedure is performed under general anesthesia, a fifty percent premium is added to the surgeon’s professional fee.
4. Hospital fees and the cost of the intraocular lens implant are billed separately and are paid directly by the patient.
5. Final surgical planning requires complete ophthalmic evaluation and diagnostics. Visual outcomes vary and are not guaranteed.
6. When applicable, PhilHealth pre-approval is required. Insurance or HMO cases require an approved letter of authority or guarantee letter before the procedure.
7. A daily hospital room rate charge applies to all admissions and is billed separately by the hospital based on room type and length of stay.

EVO VIVA ICL is an Extended Depth of Focus phakic intraocular lens that improves distance and intermediate vision. The surgeon implants this lens inside the eye without removing corneal tissue. As a result, the procedure preserves the natural cornea while delivering clear and stable vision.
WHAT EVO VIVA ICL CORRECTS
EVO VIVA ICL corrects moderate to high nearsightedness and astigmatism while extending focus for distance and intermediate activities. Patients often see more clearly when driving, using digital devices, or performing daily tasks. However, patients still need reading glasses for near work.
HOW THE PROCEDURE WORKS
The ophthalmologist performs a detailed eye evaluation to confirm eligibility. During the procedure, the surgeon creates a small corneal incision and inserts the foldable lens into the eye. The lens rests behind the iris and in front of the natural lens. This approach preserves corneal structure and allows reversibility when medically indicated.
