SPECULAR MICROSCOPY
OVERVIEW
Specular microscopy is a noninvasive eye imaging test that evaluates the innermost layer of the cornea called the corneal endothelium. This layer is made up of specialized cells that keep the cornea clear by pumping out excess fluid. The test captures highly magnified images of these cells and measures their number, size, and shape. It helps your ophthalmologist assess corneal health and determine whether the cornea can safely tolerate certain eye conditions, medications, or surgical procedures.
HOW THE TEST WORKS
Specular microscopy uses a dedicated camera and microscope system to take detailed photographs of the corneal endothelial cells. You will be asked to place your chin and forehead on a support and look at a fixation target. The device automatically focuses and captures images within seconds. No contact with the eye is required in most modern systems, and no injections or incisions are involved.
WHAT THE TEST MEASURES
Specular microscopy provides objective, computer-assisted measurements, including:

Endothelial cell density (number of cells per square millimeter)

Cell size variation (polymegathism)

Cell shape variation (pleomorphism)

Overall endothelial cell pattern and integrity
These parameters help estimate how well the cornea can maintain its clarity.

Your ophthalmologist may recommend this test for the following reasons:

Preoperative screening before cataract, refractive, or corneal surgery

Monitoring patients with corneal diseases such as Fuchs endothelial dystrophy

Evaluating corneal swelling or unexplained blurred vision

Assessing the effect of long-term contact lens wear

Follow-up after eye surgery or trauma
WHAT YOU MAY EXPERIENCE
Specular microscopy is quick and generally comfortable. Most patients complete the test in a few minutes. Bright lights may be seen briefly during image capture. Vision is not affected afterward, and you can usually return to normal activities immediately unless other tests are performed on the same visit.
