OVERVIEW
The Farnsworth D-15 Dichotomous Color Blindness Test is a clinically validated color vision screening tool that helps eye care professionals detect and classify color vision deficiencies. Clinicians use this test worldwide in ophthalmology and optometry clinics because it is fast, reliable, and easy for patients to perform. It evaluates how well a person can distinguish between similar color shades, especially along red-green and blue-yellow confusion axes.
WHAT THE TEST MEASURES
This test measures functional color discrimination rather than simple color naming. The patient arranges a set of colored caps in a natural color sequence starting from a fixed reference cap. Because the colors are close in tone, the task reveals patterns of confusion. As a result, the examiner can identify whether a color vision defect is likely present and determine its general type and severity.
HOW THE PROCEDURE WORKS
During the test, the patient views colored caps under standardized lighting conditions. The examiner first explains and demonstrates the task. Next, the patient arranges the caps in order of closest color match to form a smooth progression. The clinician then records the sequence and plots the results on a scoring sheet. Therefore, specific crossing patterns quickly show whether color confusion exists.
The procedure is noninvasive and typically takes only a few minutes per eye. Moreover, it works well for both adults and cooperative children.
WHO SHOULD TAKE THIS TEST
An eye specialist may recommend the Farnsworth D-15 test for the following:
1. Patients with suspected color vision deficiency
2. Screening for occupational color vision requirements
3. Monitoring acquired color vision changes from eye or optic nerve disease
4. Baseline evaluation before and after certain retinal or optic nerve treatments
CLINICAL VALUE AND EVIDENCE BASIS
Clinical guidelines and peer-reviewed studies support arrangement-based color tests such as the Farnsworth D-15 for practical color vision assessment. In everyday practice, clinicians often use it together with other color plates or advanced color tests. Consequently, this combined approach improves diagnostic confidence. The D-15 is especially useful for detecting moderate to severe defects that may affect daily tasks and job performance.
PATIENT EXPERIENCE
Most patients find the test simple and stress free. No eye drops, contact instruments, or bright lights are required. However, proper lighting and clear instructions remain important for accurate results. If a patient already wears prescription glasses for near work, they should use them during testing for best performance.
IMPORTANT NOTES AND LIMITATIONS
The Farnsworth D-15 test serves as a screening and classification tool, not a standalone diagnostic test. While it detects clinically significant color confusion, it may not reveal very mild defects. Therefore, the eye care professional may recommend additional color vision tests when needed. Final interpretation always depends on professional clinical judgment and correlation with the full eye examination.
