OVERVIEW
Intense Pulsed Light (IPL), Low-Level Light Therapy (LLLT), and Manual Meibomian Gland Expression are clinic-based treatments used to manage meibomian gland dysfunction and evaporative dry eye disease. These therapies aim to improve the quality of the oily layer of the tear film, stabilize tears, and reduce eye irritation. They are commonly recommended when artificial tears and home care alone are not sufficient.
These treatments are supported by peer-reviewed studies and ophthalmic practice guidelines for selected patients with chronic dry eye related to meibomian gland dysfunction.
INTENSE PULSED LIGHT (IPL)
IPL uses controlled pulses of broad-spectrum light applied to the skin around the eyelids and cheeks. The light energy helps reduce abnormal blood vessels and inflammation along the lid margin and warms the meibomian glands to improve oil flow.
Expected effects include reduced eyelid inflammation, improved tear stability, and decreased dryness symptoms. Treatment is usually performed in a series of sessions spaced weeks apart. Protective eye shields are used during the procedure.
Common temporary effects may include mild redness, warmth, or skin sensitivity. Skin burns, pigment changes, and blistering are rare but possible risks.
LOW-LEVEL LIGHT THERAPY (LLLT)
LLLT uses non-thermal, low-energy light delivered through a specialized mask placed over closed eyes. The light gently heats the eyelids and stimulates cellular activity without causing tissue damage.
This therapy is designed to soften thickened gland secretions and support healthier gland function. It is painless and typically lasts several minutes per session.
Temporary mild eyelid warmth or redness may occur. Serious complications are uncommon when proper protocols are followed.
MANUAL MEIBOMIAN GLAND EXPRESSION
Manual gland expression is performed by the ophthalmologist using controlled pressure on the eyelids to evacuate blocked or thickened gland contents. It is often done after IPL or LLLT, when oils are softened.
This step directly clears obstructed glands and allows improved oil release into the tear film. The procedure may cause brief discomfort or pressure sensation.
Possible temporary effects include eyelid tenderness, redness, or mild irritation. Small surface bruising is uncommon but can occur.
